This is one of Mel’s favorite forms. As past Employers we are often given as references about past employment. This unique form requires the departing employee to give you permission to give information concerning their employment. It also requires the employee to tell you what information you can release. Employees who were great have no problem giving you the right to release information. Those that were not so great will not give you permission or limit the permission granted. So when asked to give references all you have to say The X employee has not given us permission to release any information concerning their work history other than dates of employment. Which for a prospective employer conducting a reference check can be very revealing.
